Step 1: Complete Intake Documents

Fill out the New Patient Documents. One of our friendly staff will contact you within 1-2 business days of completion.

We will take time to understand you, your goals, and go over your lab results during the Telemedicine Visit.

Step 3: Schedule your telemedicine appointment with one of our providers

Once you've subscribed to one of our programs, your treatment will Begin Right Away! Let the journey begin!
